प्रश्नावली (Questionnaire) प्रश्नों या कथनों का समूह हैं, जिसके माध्यम से व्यक्ति से पूछकर सूचनाएं एकत्रित की जाती हैं। प्रश्नावली, अनुसन्धान करने का एक औजार है जिसमें लोगों से सूचना एकत्र करने के लिए उनसे बहुत से प्रश्न पूछे जाते हैं। इस विधि की खोज 1998 में लन्दन सांख्यिकी सोसायटी ने किया था।
